Below are enrollment forms for new students enrolling in Mason Public Schools. All new enrollees must provide the following information to be admitted into school:
State Certified Birth Certificate
Immunization Record complete with the following: DPT-4 doses, Polio-3 doses, MMR-2 doses, Hepatitis B-3 doses, Varicella- 2 doses or history of Chicken Pox, Meningococcal-1dose. This is due at enrollment and cannot be delayed until the student file arrives from the previous school.

Please complete all the necessary information and return the completed forms to the high school prior to your appointment. You must contact the high school to arrange a date and time to enroll student and to make class selections. Counseling Office (517) 676-6519
